New Arrival Physical
Every resident entering RISS is offered a physical and medical consultation at no cost during the intake process.
The purpose of this assessment is to identify current health concerns, review medications, document chronic medical conditions, and determine whether additional medical attention or follow-up care may be needed. The assessment also helps support continuity of care for residents who are managing ongoing medical conditions or receiving treatment prior to arrival.
Participation in the medical assessment is voluntary and is not a condition of residency.
However, residents are required to disclose any known medical conditions, prescription medications, allergies, treatment requirements, or other significant health concerns that may affect their health, safety, or ability to receive appropriate care while living within the community.
The assessment may include:
• Review of current and past medical conditions
• Medication review and verification
• Identification of chronic health concerns
• Discussion of current symptoms or health issues
• Recommendations for follow-up care
• Referrals to outside medical providers when appropriate
Many individuals returning from incarceration experience interruptions in healthcare, medication access, or treatment continuity. This assessment provides an opportunity to identify those needs early and connect residents with appropriate resources and support.
All medical information is handled in accordance with applicable privacy requirements and professional standards.
